When you don't trust what you're doing, you tend to tense up, tighten your grip and try to steer the ball. Think of it this way: When you strangle the club, the club doesn't choke - you do.
I want you to start monitoring your grip pressure. On a scale of 1-10, try to stay at a five all the way through - a five at address, five on the backswing, five on your downswing.
In the photos to the left, you can see the contrast. On the top, I'm strangling the club, while on the bottom, I've got a nice firm grip, but I'm not trying to choke the club to death.
I try to focus on the consistency of my grip pressure. I maintain a 5 from address through impact and follow-through for consistent, even results.

Monitor your grip pressure and arm pressure; because it will affect the way you hit the ball. Your shoulder and forearms should be free of tension; after all the more tension you possess you can expect your game to be affected in a bad way.
